Clinical positions at the Center for Disability Services are generally responsible for assessing the health care needs of, and providing treatment to the individuals we serve. The majority of these individuals are clinically complex, so strong clinical assessment skills are essential for most clinical positions.
RN Health Care Coordinator
Our Residential division is currently seeking a Registered Nurse. RN’s at the Center for Disability Services contribute to the mission by assessing health care needs and providing direct support and treatment to the people we support. Requirements include: Current NYS RN license, 1 year of acute care or RN experience preferred, and previous experience with MR/DD population.
This is a full-time Monday-Friday position with day time hours, no weekends and no holidays.
LPN Health Care Coordinator
Oversees the medical management for individuals assigned to their caseload. Provides direct oversight to consumers for health, medical issues. Regulatory compliance of required documentation. Provides oversight for direct care staff, re: topics related to and associated with maintenance of consumer’s health and safety. Must be a Licensed Practical Nurse with a minimum of 2 years experience. Must be at least 18 years old, and be able to lift a minimum of 50 pounds.
